ABOUT VITALITY HEALTH OF UTAH
Vitality Health of Utah provides mobile primary care services. This means that our providers come to you, wherever you live. If you have a primary care physician that you have been seeing, that’s ok! You can be seen by a Vitality provider and still go and see your regular primary care physician if you would like to. Having a mobile provider does not affect how or when you see your other providers.

24/7 on-call access to a provider
Comprehensive physicals and annual wellness exams, preventative care, and screening
Medication management and pharmacy coordination
Mental and behavioral health and medication management
Coordination of care between all your doctors
Hospital discharge – we will see you at home within 24-48 hours to facilitate your transition home, coordinate your care, order home health or in-home therapy, pain management, etc.
Education, discussion, and coordination of advanced planning and directives
Referral and coordination with appropriate specialists (providers that you currently see and any new specialists)
Skilled providers for dementia/Alzheimers care including testing and competency
Palliative care with a focus on the best quality of life
